REGISTRATION

Fees

  • High School Boys - $295.00
  • High School Girls - $295.00
  • Middle School Girls - $180.00
  • 7th & 8th Boys - $235.00
  • 5th & 6th Boys - $195.00
  • 3rd & 4th - $135.00

STEP 1: Forms

  1. 5/6 thru High School - Complete and turn in all forms to the appropriate school office before January 4, 2010

      Forms packet includes:

      • IHSAA Physical
      • Emergency Release
      • Physical Trainer Release
      • Parent and Student Consent Form

    • 3/4 - Complete and turn in physical only to team coach at first practice

    STEP 2: Online Registration


    SPECIAL INSTRUCTIONS FOR 3RD/4TH GRADE REGISTRATION

    Thank you for using our on line registration system to join Zionsville Lacrosse. Due to the automated application of a $75 late fee for all other programs, we have altered the 3/4 program registration fee to indicate a charge of $60, so that the overall cost to register a 3/4 player will remain at $135.00. Please be aware that the actual cost to register is $135.00 and includes the uniform. The US Lacrosse registration fee is in addition to our registration costs.

  2. All players are required to register online
  3. Registration can be completed using Visa, Mastercard, or by personal check.
  4. Each player is required to be a US Lacrosse member, you will have the opportunity to pay this membership through ZLAX. If your player's expiration date is already after June 2010 you can waive this fee.
  5. Uniform needs will be completed during this process, before starting check if your player needs to replace any uniform items from last season (shorts, practice jersey, etc).
